- Provide picture of business location
- Provide Social Security Card or Passport photocopy
- Provide copy of BTB Certificate (if a Hotel or Resort)*
- Provide copy of Business Certificate of Registration
- Provide copy of Trade License, Certificate of Incorporation, Memorandum and Articles of Association if applicant is a company
- Provide copy of Cancellation/Refund Policy
- Provide previous two years Income/Pro-Forma Statement
